Instructions:
Step_1: Fill out the form, and submit it. Please pay special attention that email addresses are correct.
Step_2: Go to your UJ email. You will have received a link for your approval. Click on that link, and electronically sign your form.
Step_3: Your supervisor will receive an email with a link for their approval. Once they approve it. You will receive an email confirming that it has been approved.
