Graduation Intent Form



  • Please print your name as you want it to appear on the diploma and in the commencement program.

  • Please print your name phonetically so we pronounce it correctly at graduation.

  • Please print your hometown and state as you want it to appear in the commencement program.

  • Please list all majors and concentrations that you will have completed. If there is a concentration, please list the concentration next to the major. (Please note that all Business Administration majors need at least one concentration).

  • Please list all minors that you will have completed.

  • It is your responsibility to make sure all requirements are met for graduation. If your advisor tells you “everything is fine,” you should still take the time to compare your transcript to the catalog and double check all required courses for your majors, minors and generals. This will help you avoid problems during your last two semesters at University of Jamestown.

    The following requirements must be met for graduation.
    • Complete all courses (with a grade of C- or higher) listed as required in the catalog for any majors with which you plan to graduate.
    • Complete all courses (with a grade of C- or higher) listed as required in the catalog for any minors with which you plan to graduate.
    • Complete all general education requirements (you may earn a grade of D- or higher for all general Education courses except English 101 and 102, which require a C-).
    • You need to earn at least 36 upper level credits (any 300-400 level course). You should carefully check your total upper-level credits earned if you have ever repeated a course.
    • Your total credits earned must be at least 124. You should carefully check your total credits earned if you have ever repeated a course.

    During the spring semester, the Registrar’s Office will keep in contact with you by sending notices to your campus email address. You will receive copies of the documents used to order your diploma and the commencement program to ensure accuracy. Therefore, you need to make sure that you read your email or stop by our office to check the accuracy of your commencement information.
